- From anywhere traveling to any physical provider in Illinois, Iowa, Kansas, Michigan, Minnesota, Nebraska or Ohio.
- From Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, North Dakota, South Dakota or Wisconsin traveling to any physical provider in the U.S..
- From anywhere traveling to specific providers in Illinois. (see below)
